Program Exclusions and Limitations:
- This program is a discount plan, not a dental insurance plan
- This plan is only honored at NewStar Dental.
- It cannot be used:
- In conjuction with another dental plan
- For services, that are covered under workman`s compensation
- For costs of dental care, which is covered under automobile medical
- There is $50 reinstatement fee if your plan lapses
- Non refundable
- Patient`s portion of bill is due day of treatment
- Terms and conditions are subject to change
